The small print reads “Jacksonville area resident and high school senior Eric Czervionke was named a ‘Scholar Athlete Milk Mustache of the Year’ (SAMMY) Winner … Congratulations, Eric.” with the address www.floridamilk.com, so this single ad was a Florida piece. The 2001 SAMMY winners ad (extra-041) prints him as “Eric Czervionke, Ponte Vedra Beach, Florida”. The title earlier spelled the name “Czerwionke”; it now follows the ad.
