A newspaper order form headed “MAC & Cheese” and “Mark McGwire “got milk?” Mustache Poster Offer”, with a small picture of the McGwire milk mustache ad. Readers were to send the form with two Hautly Cheese UPCs and $2.00 postage and handling to an address in St. Louis. It also reads “Order forms also available at Dierberg’s, Shop n Save and Schnucks Markets”, “OFFER EXPIRES OCTOBER 1, 1999” and “LIMIT ONE POSTER PER ORDER FORM PLEASE.” The form is printed twice on the page.
Issued by: “Sponsored by Hautly, Midland Dairy Assoc., Suburban Journals”, as printed. A local offer in the St. Louis area, not a national campaign piece. It names the campaign’s got milk? poster of Mark McGwire and shows it small; the form carries no campaign logo of its own.
